1. What is the Twinaqt drug and what is it used for
The drug Twinact contains two active substances that are used to treat HIV infection: dolutegravir and lamivudine. Dolutegravir belongs to a group of antiretroviral drugs called integrase inhibitors (InI). Lamivudine belongs to a group of antiretroviral drugs called nucleoside reverse transcriptase inhibitors (NRTIs).

Indications for use

Twinaqt generic Dovato is used in adults and adolescents over 12 years of age with a body weight of at least 40 kg for the treatment of HIV infection (human immunodeficiency virus).

The method of action of the drug Twinact

The drug Twinaqt does not cure HIV infection, but reduces the amount of the virus in your body and keeps it at a low level. The drug also increases the number of CD4+ cells in your blood. CD4+ cells are a type of white blood cells that help your body fight infection.

Not all people respond to treatment with Twinact in the same way. Your doctor will monitor the effectiveness of your treatment.

If there is no improvement or you feel worse, you should consult a doctor.

2. What you should know before taking Twinaqt
Contraindications

Do not take the drug Twinact generic Dovato:

if you are allergic to dolutegravir, lamivudine or any other components of the drug (listed in section 6 of the leaflet);
if you are taking fampridine (also known as dalfampridine; used to treat multiple sclerosis).
If you think that any of the listed cases applies to you, consult your doctor.

Special instructions and precautions

Before taking the drug Twinact, consult with your doctor.

Some people taking the drug Twinaqt or other types of combined treatment for HIV infection are at a higher risk of serious side effects. You need to be aware of additional risks:

if you have moderate or severe liver disease;
if you have ever had liver disease, including hepatitis B or C (if you are infected with hepatitis B virus, do not stop taking the drug Twinact generic Dovato without consulting a doctor, because you may have a recurrence of hepatitis);
if you have kidney problems.
➔ Before using Twinaqt, inform your doctor if any of the above applies to you. You may need additional examinations, including blood tests, throughout your treatment with Twinaqt. For more information, see section 4.

Allergic reactions

The drug Twinaqt contains dolutegravir. Dolutegravir can cause a serious allergic reaction (hypersensitivity reaction). You need to know about the important signs and symptoms that you should pay attention to while you are taking the drug Twinact.

Pay attention to the important symptoms

Some people taking drugs for the treatment of HIV infection develop other pathologies that can take a serious character. These include:

symptoms of infections and inflammations;
joint pain, joint stiffness and bone problems.
You need to know about the important signs and symptoms that you should pay attention to while you are taking the drug Twinact.

Protect other people

HIV infection is transmitted through sexual contact with an infected person or through infected blood (for example, when using common needles for injection). You can still transmit HIV infection to other people when taking the drug Twinact, although this risk is reduced thanks to effective antiretroviral therapy. Discuss with your doctor the precautions necessary to prevent infection of other people.

Children

Do not give the drug to children from 0 to 12 years old and weighing less than 40 kg due to the risk of inefficiency and possible insecurity (the safety and efficacy of the drug Twinact in children under 12 years of age have not been established).

Other drugs and the drug Twinact

Tell your doctor that you are taking, have recently taken, or may start taking any other medications.

Do not take Twinaqt with the following medication:

fampridine (also known as dalfampridine; used to treat multiple sclerosis).
Some medications may affect the effect of the drug Twinact or increase the likelihood of adverse reactions. The drug Twinact may also affect the effect of some other medications.

Tell your doctor if you are taking any of the following medications:

metformin (for the treatment of diabetes mellitus);
antacid drugs (for the treatment of digestive disorders and heartburn). Do not take antacids for 6 hours before taking the drug Twinact or at least for 2 hours after taking it (see also section 3 "Taking the drug Twinact");
dietary supplements and multivitamins containing calcium, iron or magnesium. If you take the drug Twinact with a meal, you can take dietary supplements or multivitamins containing calcium, iron or magnesium at the same time as the drug Twinact. If you do not take Twinact with meals, do not take dietary supplements and multivitamins containing calcium, iron or magnesium for 6 hours before taking Twinact or at least 2 hours after taking it (see also section 3 "Taking Twinaqt");
emtricitabine, etravirine, efavirenz, nevirapine or tipranavir/ritonavir (for the treatment of HIV infection);
medications (usually liquid) containing sorbitol and other sugar alcohols (such as xylitol, mannitol, lactitol or maltitol) if you take them regularly;
cladribine (for the treatment of leukemia or multiple sclerosis);
rifampicin (for the treatment of tuberculosis and other bacterial infections);
phenytoin and phenobarbital (for the treatment of epilepsy);
oxcarbazepine and carbamazepine (for the treatment of epilepsy and bipolar disorder);
St. John's wort (Hypericum perforatum; herbal preparation for the treatment of depression).
Tell your doctor if you are taking any of the above medications. Your doctor may decide on a dose adjustment or the need for additional examinations.

Pregnancy and breastfeeding

If you are pregnant or breastfeeding, think you are pregnant, or are planning a pregnancy, consult your doctor before using the drug.

Pregnancy

Taking Twinaqt during conception or during the first six weeks of pregnancy may increase the risk of congenital malformations called neural tube defects, such as spina bifida (spinal cord deformity).

If you can get pregnant while taking the drug Twinact:
Consult with your doctor and discuss the need to use contraceptives, such as condoms or pills.

Tell your doctor immediately if you become pregnant or are planning a pregnancy. Your doctor will review your treatment. Do not stop taking the drug Twinact without consulting a doctor, as this may harm you and your unborn child.

Breastfeeding

HIV-infected women should not breastfeed their baby, because HIV infection can be transmitted to the baby with breast milk.

A small amount of the components that make up the drug Twinact can also penetrate into breast milk.

If you are breastfeeding or considering breastfeeding:
➔ Consult your doctor immediately.

Driving vehicles and working with mechanisms

The drug Twinaqt can cause dizziness and drowsiness, as well as other side effects that negatively affect your attentiveness.

➔ Do not drive vehicles and do not work with mechanisms until you are sure that you do not have such effects.

3. Taking the drug Twinact
Always take the drug Twinact in full accordance with the recommendations of your attending physician. If in doubt, consult with your doctor.

Recommended dose

The recommended daily dose for adults and adolescents over 12 years of age with a body weight of at least 40 kg is 1 tablet 1 time a day.

Antacid drugs

Antacid drugs used to treat dyspepsia and heartburn can interfere with the absorption of the drug Twinact in the body and reduce its effectiveness.

Do not take antacid medications for 6 hours before taking Twinact or at least for 2 hours after taking it.

Consult your doctor for additional recommendations about taking acid-reducing medications at the same time as Twinaqt.

Dietary supplements or multivitamins containing calcium, iron or magnesium

Calcium-, iron- or magnesium-containing dietary supplements or multivitamins can interfere with the absorption of the drug Twinact in the body and reduce its effectiveness.

Do not take medications containing calcium, iron or magnesium for 6 hours before taking the drug Twinact or at least for 2 hours after taking it. If you take the drug Twinact with food, you can take dietary supplements or multivitamins containing calcium, iron or magnesium at the same time as the drug Twinact.
