Tofamark is a medicine supplied as film-coated tablets. The tablets are white or almost white and have a biconvex shape. Each pack contains 60 tablets and instructions in several languages.

Pharmacological action

Tofacitinib can inhibit Janus kinase (JAK) with high selectivity compared with other kinases in the human genome. The medicine helps regulate the immune response and slow the development of inflammation, which contributes to effective control of disease symptoms.

Pharmacodynamics

The medicine blocks signal transmission through heterodimeric receptors by inhibiting JAK1, JAK2, JAK3 and tyrosine kinase 2. It also affects interleukin-6 (IL-6) and interferon-gamma (IFN-gamma), weakening erythropoietin signal transmission. When used at recommended doses, complex changes in the immune response are observed and may persist after therapy is completed.

Pharmacokinetics

After oral administration, the tablet is rapidly absorbed and reaches maximum concentration within 30-60 minutes. The medicine is mainly metabolized in the liver (70%) and is partly excreted unchanged by the kidneys (30%).

Indications for use

The medicine is recommended for the treatment of patients with moderate or severe rheumatoid arthritis when baseline disease-modifying medicines do not provide the desired effect.

Contraindications

Tofacitinib is contraindicated in the following conditions:

  • Minor age;
  • Severe local and systemic infectious diseases;
  • Use of medicines that may combine with tofacitinib and provoke infections, including interleukin-6 and interleukin-1 antagonists, other strong immunosuppressants and tumor necrosis factor blockers.
